Dr Jacobs qualified in Medicine in 1998. In 2003, he passed the membership examination of the Royal College of Physicians, London. He was appointed as a Consultant Acute and General Physician in 2011 at Barnet Hospital, Hertfordshire. The hospital merged with the Royal Free London NHS Foundation Trust in 2014.

He sees a wide range of acute presentations of medical illness, and runs the Ambulatory Emergency Clinic, which is a streamlined way of managing patients presenting to hospital who would traditionally be admitted. Instead, they can be treated and discharged the same day.

He has been a lead investigator of Serious Untoward Incidents, using Root Cause Analysis to identify care and service delivery issues. He was a Clinical Advisor to the National Confidential Enquiry into Patient Outcomes and Deaths (NCEPOD), and has peer reviewed cases for NCEPOD’s sepsis, acute non-invasive ventilation, heart failure and pulmonary embolism studies.

He was recently appointed to the role of Medical Examiner. This role involves the independent scrutiny of medical records to establish causes of death, liaising with the clinical team involved with the care of the deceased, and the deceased’s family. Coroner referrals are made when necessary and clinical governance/organisational learning issues are also flagged up as and when appropriate.

Dr Jacobs is an examiner for the Royal College of Physicians and University College London Medical School. He has presented work at international conferences, published articles in peer-reviewed journals and participates in the peer review process of submitted manuscripts.

Cases undertaken

All aspects of acute and general medicine. Special areas of interest include sepsis, acute kidney injury and DVT/pulmonary embolism.
